146 GOLD STANDARD GOAL-SETTING: ARE WE HITTING THE MARK? IMPLEMENTING AN INTERDISCIPLINARY GOAL-SETTING PATHWAY IN STROKE REHABILITATION

Abstract Background The cornerstone of rehabilitation is effective and realistic interdisciplinary goal-setting with patient family engagement across the continuum recovery. Royal College Physicians National Clinical Guidelines for Stroke (2016) highlights importance encouraging self-management self-efficacy to support a more person-orientated approach. This quality improvement initiative aimed introduce structured pathway on post-acute 14-bed stroke unit. Methods An working group was established comprising Physiotherapy, Occupational Therapy, Speech & Language Medical Social Work Nursing. Quality methodology used sequentially implement change over 6-month period. restructure included introduction an initial review goal-focused meeting. tool developed based East Kent Outcome System (EKOS). A series workshops delivered implementation. Results currently being piloted 6 months To date, 22 staff have been trained 9 patients progressed through updated pathway. Analysis has provided useful clinical insights qualitative feedback collated regarding benefits early opportunity meet team, establishing patient’s personal goal priorities clear timeframes.Furthermore, repeat trials followed by reflections enabled team develop consistency confidence in person-centred, goal-setting. It acknowledged that this significant process adjustments are made, line principles improvement. Conclusion evidence-based national standards guidelines rehabilitation. Formal training EKOS planned audit will follow pilot phase systematically evaluate practice.
